The reconstruction of the peri-implant papilla poses a significant challenge in
implant dentistry. Teeth deemed hopeless often exhibit clinical attachment loss
between adjacent teeth, complicating implant therapy, especially in aesthetic
areas. Inadequate management of hard and soft tissues post-extraction
frequently leads to suboptimal aesthetic outcomes for dental implants,
manifesting as deficiencies in midfacial peri-implant soft tissue margins and
interproximal soft tissues, often resulting in black triangles. Both clinician
and patient expectations for dental implants have markedly risen in recent
years, magnifying even minor peri-implant papilla deficiencies in the anterior
region as compromised clinical outcomes.
This presentation will focus on ridge augmentation using guided bone
regeneration in challenging clinical scenarios where patients present with
interproximal bony peak deficiency. The discussion will delve into the use of
biomaterials, membranes, and growth factors such as ABBM, perforated d-PTFE,
collagen membranes, EMD, and PDGF-BB for vertical ridge augmentation and
interproximal bone reconstruction. Techniques for subsequent soft tissue
augmentation, including the Iceberg and Garage connective tissue graft methods
for inter-implant papilla reconstruction and enhancing papilla height
post-vertical ridge augmentation, will be introduced. Long-term outcomes
utilizing these techniques will also be presented.
